Submit these forms along with payment of the non-refundable registration fee ($50 per child) and any weeks you wish to pay for now, to the address below.   It is strongly recommended that you also pay for your child(ren)'s first week of camp at this time.  Checks or money orders should be made payable to The Study Institute.   If you wish to pay by credit card (Visa, MasterCard or American Express), then be sure to provide the complete number, expiration date and signature on the Weekly Reservation form.  Note that the Study Institute will add on a 2% processing fee for each credit card transaction.  This registration packet can be submitted in person to the Westside Extension office, located in the lobby of the PEC South building at West Los Angeles College.  (click here for map and directions and here for  Westside Extension Office Hours).
